Position Title: Head Start Health and Nutrition Coordinator

Reports To: Director of Engagement Health & Nutrition

Classification: Salaried / Exempt

Position Summary:

Position Summary:

The Health and Nutrition Coordinator plans and collaborates with other coordinators and staff in the Head Start program to ensure the health and well-being of all children in the Head Start program. The Health and Nutrition Coordinator will be evaluated on a combination of skills, dispositions, and behaviors, some of which are related to the specific service area of this position and others that represent general expectations of all Childcare Resources employees.

Primary Responsibilities:

1. Work in collaboration with the Director of Family Engagement Health and
Nutrition to develop, implement and refine Childcare Resources’ Family
Engagement/Health approach in order to promote family outcomes that support children’s school readiness and well-being

2. Coach and provide supervisory guidance to the Health Assistant in order to strengthen staff capacity and move staff towards mastery in core competencies included in the Health and Nutrition Success Rubric

3. Build a team that works collaboratively with Early learning and Family

Engagement staff in order to enhance family life practices and circumstances that promote child development and well-being

4. Maintain qualitative and quantitative quality improvement and monitoring systems in order to assure that program strategies are executed with reliability, fidelity and quality

Health and Nutrition Services: Integrates knowledge of evidence-based practices to promote child and family health and wellness to build a health and nutrition program that:

1. Helps staff and families understand the relationship between child and family health and nutrition and children’s school readiness outcomes

2. Develops strategies to address the health disparities in low income communities, particularly related to common chronic health conditions such as asthma, obesity and lead exposure

3. Engages families as leaders in building a program environment that promotes health and wellness

4. Oversees completion and follow up of yearly nutrition assessments for all children in the program, including providing guidance, support and follow-up to develop Integrated Care Plans for children with special nutritional needs

5. Assures that all children with special nutritional needs have individualized diets and special food, as needed

6. Develops community partnerships to assure that children and family’s preventive and health and nutrition treatment needs are met; and that secures long-term relationships for families to providers who will support their children’s long term health and well-being

The Health and Nutrition Coordinator’s responsibilities chiefly fall under the following core functions, although s/he may perform other tasks as needed:

9. Record Keeping and Reporting


1. Assure that all children in the program are up to date on EPSDT preventive health care, requirements, and receive timely follow up evaluation and treatment

2. Assure that program has documentation or has conducted health screenings for all children within HS Performance Standards 45 and 90 required time frames

3. Establish monitoring systems to oversee EPSDT adherence, including follow up on expired or coming due to preventive health visits and follow up

1. Provide oversight, guidance, and training to program staff to complete and interpret HS Performance Standard required Nutritional Assessments within 90 days of a child’s program entry

2. Coordinate with Food Services team to develop specialized diets for children with dietary restrictions and assure that all staff understand children’s special needs

3. Coordinate with Health and family engagement team to conduct Integrated Care
Plan meetings for families of children with specialized nutritional needs, such as children who are underweight, obese, or diagnosed with anemia

4. Provide individualized support to families of children with significant nutritional needs, including regular care conferences, care coordination, and family education

5. Coordinate with Service area teams to develop and implement health and wellness strategies to reduce incidence of childhood obesity

1. Establish trusting and respectful family relationships with diverse families in order to effectively engage them in maintaining ongoing preventive care and follow up to assure their children’s health and well-being

2. Effectively communicate the relationship between children’s health and nutrition and school readiness outcomes

3. Support families to become advocates for their children’s health, with special attention to families of children with chronic health conditions


1. Assure that all families have quality medical and dental homes, and maintain consistent health insurance coverage, if eligible

2. Complete Individual Care Plans for children with chronic health conditions, and follow up to ensure that children’s health conditions are effectively managed

3. Develop and align program-wide health and wellness campaigns with agency mission; including promoting consistent family routines that support healthy lifestyles and habits

4. Work with program-wide leadership to build staff and agency health and wellness culture to model and support our health and nutrition goals for children and families


1. Effectively coordinate with family engagement, Early Learning, and MH and
Disabilities teams to maintain health and nutrition requirements documentation and follow up

2. Coordinate goal setting with family engagement teams to assist families to strengthen chronic health condition management and service access

3. Communicate and coordinate with Center staff, families and community partners to ensure that children’s health and nutrition needs are understood and that individualized support is in place when needed

1. Assists with establishing and maintaining community partnerships and collaborations to secure health and nutrition screenings and follow up treatment, including follow up on Memoranda of Understanding

2 Facilitates the organization of the Health Service Advisory Committee and utilizes the committee in the planning, implementation and evaluation of the health component.

3 Coordinates as appropriate with School District health staff


1. Assure that all classrooms and centers are in compliance with municipal, state and federal health and safety practices, including ongoing monitoring

2. Oversee medication administration, storage and auditing to maintain up to date information, medications and medical authorizations

3. Assure that all classrooms have up to date food allergy lists that include food substitutions for children with special diets

4. Establish effective health emergency procedures, including conditions for short-term exclusion and admittance; medication administration procedures; injury prevention measures; and hygiene procedures

5. Coordinate with Operations team to assure that facilities meet health and safety compliance regulations


1. Utilize program data to plan and monitor health and nutrition services, including developing systems to assure that all children remain up to date on EPSDT requirements, evaluation and follow up treatment

2. Utilize health and nutrition trend data, community assessment, and other community indicators to inform health and wellness planning, and to target specific interventions to at risk children and families

3. Utilize health and nutrition trend data to identify areas for professional development for staff, as well as to develop health promotion and education campaigns for families

1. Maintain real time up to date accurate documentation of all health and nutrition requirements and health and nutrition screenings

2. Assure that all child files are 100% in compliance with HSPS, licensing Childcare

Resources policies and procedures
3. Effectively manage workload to assure that core job functions are completed in timely manner


1. Bachelor’s Degree Required (Food and Nutrition or Health degree preferred)

a. Master’s in Nutrition Sciences or MPH or comparable degree preferred.

b. Registered Dietician or Nutritionist preferred

2. Experience working with families of children 0-5 in low-income communities supporting their health, nutrition, and wellness goals

3. Experience administrating CACFP (Child and Adult Care Food Program)and Early Childhood Experience Preferred

4. Training and Experience in public health, nutrition sciences, nursing, health education, maternal and child health, or health administration

5. Physical exam and background checks are required for this position
6. Travel required locally or long-distance up to 10% of the time for work-related meetings and functions
6. Must have a valid driver’s license and reliable transportation

Physical Requirements:

Work Environment:

1. Work is generally performed in an office environment
