The Greater Rochester Health Home Network (GRHHN), a subsidiary of FLPPS, is a Health Home (HH) serving the most vulnerable people in our community, especially those struggling with multiple medical and behavioral health conditions. GRHHN works closely with Care Management Agencies (CMA) under the care management service model to ensure communication among all of an individual's caregivers with the goal of addressing all of the patient’s needs in a comprehensive manner.

The Finger Lakes Performing Provider System (FLPPS) is a network of clinical and community-based providers working together across the 13-county Finger Lakes region to transform healthcare delivery, advance system transformation, close gaps in care, and support high-risk individuals and populations. FLPPS and our affiliated health home, the Greater Rochester Health Home Network (GRHHN) support providers in integrated healthcare delivery system optimization, care management, network and program management, population health and data analytics, and learning and development.

POSITION SUMMARY

Provide Supervision to a team of Care Managers in the development and implementation of care coordination and care management services for high-risk Client populations to ensure efficiency and effectiveness in their care. Work in partnership with Senior Leadership to implement strategies that serve Providers and Clients with excellence in care and service delivery.

In addition to supervisory responsibilities this position will directly support the Sr Manager. They will support the leadership team in meeting or exceeding performance standards.

RESPONSIBILITIES

Supervisory Responsibilities

· Provide supervision and support to a team of Care Managers.

· Provide oversight and direction of the daily workflow for the Care Management team; ensuring timely and appropriate plans of care are developed and executed

· Ensure Care Managers meet or exceed NYS Documentation Requirements for Care Plans.

· Assure a coordinated approach to the community at large and support better health, better quality of care and lower costs.

· Review and sign off on lost engagement clients, diligent search efforts and discharges.

· Complete monthly internal audits.

· Adhere to responsibilities related to providing care management.

· Assist Sr. Manager in ensuring Care Managers meet or exceed NYS Documentation Requirements for Care Plans.

· Collaborate with Senior Manager in creating care manager performance reviews

· Support relationships and work collaboratively with the clinical community and non-clinical community agencies to support implementation of plans of care that reduce inpatient admissions and emergency department usage.

· Assist Sr. Manager in developing activities for continuous process improvement for assessment, engagement, planning and implementation of goals for the plans of care.

· Carry a small or Interim caseload as needed based on program needs.

Case Review and Collaboration

· Facilitate Care Team meetings: schedule, prepare and participate in care team meetings to share discoveries, concerns and collaborate in the development of plans of care.

· Ensure Care Team meeting output is communicated to the client and all consented care team members.

· Contribute to the GRHHN Care Management team to foster positive work relationships and support team members for cross coverage as workload dictates.

  • Collaborate with other staff related to client needs, barriers to care and outcome enhancement strategies.

QUALIFICATIONS

· A Bachelor of Arts or Science with 5+ years of relevant experience, or;

· A license as a Registered Nurse with 5+ years of relevant experience, or;

· Master’s degree in social work or RN/LPN or equivalent experience

· Minimum of 5 years’ experience supervising teams in a health care environment.

· Knowledge and experience with NYS Health Home Program

· Demonstrated history of compliance with all HIPAA Privacy and Security Standards

· Knowledge and experience working with community-based organizations and resources.

· Knowledge of and ability to work collaboratively with providers and county/community health and human services.

· Proficiency with Microsoft Office Suite (Word, Excel, Outlook).

· Ability to learn new software; Knowledge of data analytics tools and dashboards to support analysis and reporting.

· Must have and maintain an unrestricted NYS driver’s license and insured vehicle for frequent/daily travel in the community travel.

· Bilingual skills, preferred.
