Position Summary

The PrEP & Health Navigator supports HIV prevention and harm reduction efforts by providing outreach, education, counseling, and linkage to care for HIV, HCV, and STI services. This role works closely with communities most impacted by health disparities—including LGBTQIA+ individuals, communities of color, and people affected by substance use—to increase access to prevention tools such as PrEP/PEP and ongoing care.

What You’ll Do

Outreach & Engagement

  • Conduct street, community, clinic-based, and digital outreach to connect with individuals at risk for HIV/HCV/STIs
  • Represent the organization at community events, workshops, and outreach campaigns

Education & Prevention

  • Provide education on HIV/HCV/STIs, PrEP/PEP, U=U, sexual health, and harm reduction
  • Deliver risk-reduction counseling using motivational interviewing techniques
  • Conduct HIV/STI testing (or obtain certification upon hire)

Care Navigation & Support

  • Help clients access medical care, including scheduling appointments and referrals
  • Assist with insurance navigation and enrollment in medication assistance programs
  • Support clients in starting and staying on PrEP, PEP, or other treatments
  • Maintain ongoing follow-up to ensure engagement and adherence

Collaboration & Documentation

  • Work with medical, behavioral health, and prevention teams to coordinate care
  • Maintain accurate and timely documentation in electronic systems
  • Attend trainings, team meetings, and supervision sessions

What We’re Looking For

Required Qualifications

  • Bachelor’s degree in Public Health, Social Work, Psychology, or related field (or equivalent experience)
  • 1–2 years of experience in community health, HIV prevention, harm reduction, or case management
  • Knowledge of HIV/STI prevention, PrEP/PEP, and sexual health
  • Strong communication and interpersonal skills
  • Experience working with diverse communities, including LGBTQ+, BIPOC, and individuals impacted by substance use or mental health challenges
  • Proficiency in Microsoft Office and data entry systems
  • Valid California Driver’s License, reliable transportation, and auto insurance

Additional Requirements

  • Must be eligible to work in the U.S.
  • Must have at least one (1) year of recovery or maintain a drug-free lifestyle
  • Ability to complete a physical exam and TB test upon hire

Preferred Qualifications

  • Experience working in Los Angeles County public health or harm reduction programs
  • HIV test counselor certification (or willingness to obtain within 3 months)
  • Bilingual or bicultural (Spanish or other LA County threshold languages preferred)
  • Experience with trauma-informed care and harm reduction practices

PHYSICAL REQUIREMENTS

Moderate Duty:
This position requires working in both office and community settings, remaining stationary for extended periods (approximately 50% of the time), frequent communication with clients and coworkers, and the ability to exchange accurate information. Regular bending, stooping, or squatting may be required, as well as occasional lifting up to 25 pounds and ascending/descending stairs or ladders.
