Horizons Community Care of Holiday Health Care offers private duty nursing and treatment for chronic and acute illnesses. In addition to nursing/caregiving for pediatric and adult patients with disabilities. Join our growing team of Registered Nurses and Licensed Practical Nurses!

RN POSITION SUMMARY:

Provide quality, skilled nursing in accordance with the orders of a physician and under a plan of care in a patient’s home. Has the responsibility for the observation, assessment, nursing diagnosis, counsel, care, and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. Rate of pay will range based on experience .

RN POSITION QUALIFICATIONS:

  • Must be a graduate of a registered nursing program from an accredited school.
  • Must possess and maintain a current, and in good standing, license with the Indiana State Board of Nursing as a licensed registered nurse.
  • Case management requires a minimum of 2 years of satisfactory medical or surgical experience.
  • Must be a licensed driver with an automobile that is insured in accordance with State and /or organization requirements and is in good working order.
  • Must maintain current CPR certification.

  • Must be a graduate of a LPN program from an accredited school.
