Full job description

Certified Nursing Assistant (CNA)

Position Description:

The Home Health CNA is a paraprofessional member of the Home Health Team who works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patient’s needs as permitted under state law. The Home Health CNA is responsible for observing patients, reporting these observations and documenting observations and care performed.

Essential functions of this position are:

  • Providing personal care including: o Personal hygiene activities o Changing bed linen o Dressing and undressing patients o Skin care and prevention of skin breakdown o Toileting activities o Keeping patient’s living area clean and orderly, as appropriate
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure as ordered (within training limitations of the aide.);
  • Assisting in ambulation and exercise according to the plan of care;
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (within the training limitations of the aide);
  • Maintaining basic infection control procedures;
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, restraints, etc.);
  • Reporting on patient’s condition and significant changes to the assigned supervisory nurse;
  • Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct, and;
  • Other duties assigned by supervisory RN or Home Health Director.

Qualifications:

  • Must be 18 years of age;
  • High School Graduate or equivalent;
  • Has successfully completed an Illinois approved CNA training program or equivalency program.

Additional Skills, Knowledge and Abilities:

  • Possess and maintains good physical stamina and mental health.
  • Licensed driver with automobile that is insured in accordance with the organizations requirements and is in good working order;
  • Has a working telephone;
  • Has current CPR certification;
  • Self-directing with the ability to work with little direct supervision;
  • Has the ability to read and follow written instructions and document care given;
  • Requires ability to utilize good judgment, with confidentiality in execution of all duties
