HORIZON'S COMMUNITY CARE RN POSITION SUMMARY:

Provide quality, skilled nursing in accordance with the orders of a physician and under a plan of care in a patient’s home. Has the responsibility for the observation, assessment, nursing diagnosis, counsel, care, and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. Rate of pay will range based on experience.

HORIZON'S COMMUNITY CARE RN POSITION QUALIFICATIONS:

• Must be a graduate of a registered nursing program from an accredited school.
• Must possess and maintain a current, and in good standing, license with the Indiana State Board of Nursing as a licensed registered nurse.
• Case management requires a minimum of 2 years of satisfactory medical or surgical experience.
• Must be a licensed driver with an automobile that is insured in accordance with State and /or organization requirements and is in good working order.
• Must maintain current CPR certification

HORIZON'S COMMUNITY CARE LPN POSITION SUMMARY:

Provide quality, skilled nursing in accordance with the orders of a physician and under a plan of care in a patient’s home. Will participate in care coordination; has the responsibility for the observation, assessment, nursing diagnosis, counsel, care, and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. Rate of pay will range based on experience

HORIZON'S COMMUNITY CARE LPN POSITION QUALIFICATIONS:

• Must be a graduate of a LPN program from an accredited school.
• Must possess and maintain a current, and in good standing, license with the Indiana State Board of Nursing as a licensed practical nurse.
• Must be a licensed driver with an automobile that is insured in accordance with State and /or organization requirements and is in good working order.
• Must maintain current CPR certification.
