Our territory covers the following counties: Worcester, Middlesex, Norfolk, & Bristol. Home office is in Marlborough, MA

The Admissions RN is a registered nurse whose primary responsibility is to present Faith & Family Hospice services and philosophy to patients and families who reside in facilities, hospitals, long-term care communities, and private residences, in a manner that results in the admission of appropriate patients.

  • Participation in the information gathering, analysis, and review of medical records, facility charts, conversations, etc. and evaluate the patient to determine hospice eligibility.
  • Provide teaching, education, and support services available to the patient and family.
  • Facilitate the completion of consent forms to initiate the admission to hospice services for those patients who meet admission criteria.
  • Work closely with Medical Directors to obtain admission orders.
  • Collaborate with other members of the IDG team to coordinate an effective care plan in accordance with patient and family goals.
  • Build effective referral source relationships to support the high quality and responsiveness expectations.
  • Maintain clinical and professional competence.
  • Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
  • Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice and palliative care.
  • Review patient charts in Skilled Nursing Facilities to ensure they are up to date and in compliance with Medicare Guidelines. Oversee the replenishment of updated documentation in the charts on a bi-weekly basis.
  • Patient visits which include symptom management and physical assessments as requested
  • Determine care plan needs and initiating the plan of care as well as assessing the home care needs (DME, comfort kit meds, any immediate med needs).


  • Belief in the Hospice Philosophy.
  • Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas.
  • Ability to communicate effectively with patients and families, physicians, referral sources and colleagues.
  • Ability to drive throughout the various counties within our territory.


  • Currently licensed as a R.N. in Massachusetts and license in good standing.
  • Strong Presentation and organizational Skills.
  • Previous sales or service experience preferred.
  • Hospice experience preferred.
  • Reliable with car with valid insurance policy.


  • Bachelor’s degree preferred
