GENERAL DESCRIPTION:
The Housing and Health Case Manager position is responsible for providing client-centered care for Freedom House Detroit clients. This position provides assessment, care planning, and referral services to individuals and families. This is a critical position for all residents at Freedom House Detroit, ensuring the clients attain housing and independence. It will require flexibility, creativity, and the ability to follow grant-based requirements. This role reports directly to the Supervisor of Case Management Services and works closely with all program teams.
MINIMUM REQUIREMENTS:
- Bachelor’s degree in social work or a minimum of 4 years experience.
- Experience and familiarity with immigrant populations or lived experience are preferred.
- Bilingual preferred (high-need languages: French, Spanish, Portuguese, and Arabic)
- Ability to effectively communicate in writing, verbally, and electronically.
- Proficiency with Google Suite.
- Valid driver’s license and State of Michigan background check
ESSENTIAL JOB FUNCTIONS:
- Administer written individual and family assessments.
- Develop and update person-centered care plans.
- Identify client needs and consult with team goals setting.
- Maintain up-to-date client files electronically.
- Prepare reports as requested.
- Assist with all aspects of moving into permanent housing.
- Enroll children in school and assist parents in learning and navigating the school system.
- Across funder databases, maintain data related to case notes, goals planning, and the client’s case overall.
- Collaborate with the case management, legal, and site teams to coordinate services.
- Participate in all resident house meetings.
- Assist in maintaining and expanding the resource database through networking with other professionals.
- Attend scheduled work conferences, staff meetings, agency functions, and fundraisers as requested.
- Facilitate community presentations to expand community awareness of refugees, victims of torture, and Freedom House Detroit’s programs and services.
- Represent the organization at public forums, conferences, and media events; function as an advocate representing the organization in the community.
- Assist in the marketing of agency programs and services.
- Assist supervisor as needed.

- Evening and weekend work is sometimes required to meet client needs.

- Ability to work effectively with clients, caregivers, staff, volunteers, and external contacts to build and maintain effective teams.
- Ability to prioritize and manage multiple priorities.
- Problem-solving skills.
- Ability to travel as needed to perform job duties.
- Ability to operate designated office equipment.

Pay: $45,000.00 - $50,000.00 per year
