Lead Medical Provider - Family Medicine/Addiction Medicine MD/DO
$305K+ and $100K Bonus, HRSA Loan Forgiveness opportunities
Location: Southern Oregon
Scope: Full-Spectrum Family Medicine + OB & Substance Use Disorder care
Why This Role Is Different
This is more than a Medical Provider role—it’s a chance to redefine how care is delivered to families impacted by substance use disorders.
You will build and lead a family medicine clinic embedded in a residential treatment campus where families recover together, stay together, and thrive together.
You will also lead an established family medicine/addiction medicine practice, the Oasis Center of the Rogue Valley, giving you immediate stability alongside the opportunity to innovate and grow something truly unique.
What You’ll Do
- Provide full-scope family primary care, including OB, integrated with behavioral health, substance use disorder treatment, and social services to support families affected by SUD
- Launch and lead a new integrated family medicine clinic co-located on the Continuum Family Treatment campus (Fall 2026)
- Treat parents and children together, addressing multi-generational health needs
- Provide leadership and oversight for a team of providers
- Oversee mobile and off-site care at Continuum’s rural facilities

- Starting Salary: $305,000+
- $100,000 Signing Bonus

The Cobblestone Village Family Treatment Campus will provide holistic family-centered treatment that is trauma-informed, accessible, and culturally responsive. The campus will reduce barriers to care by providing childcare, behavioral health treatment for children and parents, peer support, and medical treatment on one campus. About the Oasis Center of the Rogue Valley
The Oasis Center opened in 2019 as medical home for families affected by substance use disorder. Oasis is a Patient Centered Primary Care Home (PCPCH), a Nurture Oregon site through Oregon Health Authority integrating SUD treatment with prenatal/postpartum and infant care and is a rotation site for the Oregon Health & Science University (OHSU) Addiction Medicine Fellowship and Family Medicine Residency. The Oasis care team includes medical and behavioral health providers, community health workers, peers, and doulas integrated with social support services including housing, childcare, medical-legal partnership, community multidisciplinary team, and employment support.
