This salaried position comes with a sign-on bonus up to $5,000, per aligned to one year of commitment! Eligible for every other Friday off after completing the 90-day probationary period and meeting productivity goals.

We are seeking a full-time, salaried Licensed Social Work Care Coordinator to promote and protect the health and well-being of the Medicaid Eligible Individuals of the General Care Program in Madison County. The schedule is Monday-Friday (8am-5pm). Candidate must have minimum of a Licensed Baccalaureate Social Worker (LBSW) from an accredited college. Candidate must also have a current Alabama Driver’s License and Auto Insurance (State minimum requirements).

The following are also requirements:

• Bachelor of Science in Social Work minimum
• Active Alabama license in good standing as a Licensed Baccalaureate Social Worker (LBSW)
• Experience working in a home setting as a Case Manager or Care Coordinator is preferred
• Excellent Computer/Communication Skills
• Pleasant and cooperative attitude

Essential Functions

• Complete psychosocial assessments.
• Develop a team approach Care Plan including the eligible individuals and the DHCP and other community resources and agencies.
• Complete face to face visits with clients to build rapport and identify services or referrals needed to work towards goal attainment.
• Ensure the eligible individual has access to covered services, proper referral, and resources for the General Care program.
