LPN (Licensed Practical Nurse)

Established in 1948, the Mission of ASCEND Mental Wellness is to encourage and assist individuals in discovering pathways to their optimal mental health and wellness, while supporting hope and awareness of well-being in our community.

ASCEND is seeking a Licensed Practical Nurse (LPN) to work in various program sites within our agency (Travel required). Must have current licensure as a Licensed Practical Nurse by New York State. Continued employment is dependent upon retention of licensure. Must have at least one year of clinical experience, preferably in mental health.

A $1,000 sign-on bonus is available for this position.

Responsibilities include, but are not limited to:

• Assumes self-responsibility for all assigned client care activities related to client’s medications and health based on care plans developed by the Agency RN
• Relies on experience and judgment and a certain degree of creativity and latitude to plan and accomplish goals as developed with supervisor and individual programs
• Attend hospital discharges as needed
• Assists in coordination and implementation of health education for staff and clients as necessary
• Assist with the Clinic’s Psychotropic Medication Injection service as necessary
• Assist with the Clinic’s Health Monitoring program as necessary
• Performs a variety of tasks including administrative duties such as completing medical forms, reports, evaluations, charting, etc., as necessary, and ensures the accuracy and completeness of clinical and data required documentation
• Transport and Advocate for clients during medical appointments as needed

Qualifications:

• Current licensure as a Licensed Practical Nurse by New York State. Continued employment is dependent upon retention of licensure. ·
• At least one year of clinical experience, preferably in mental health ·
• Valid NYS driver’s license acceptable to Agency insurance standards. ·
• Must be of good character and background and display an affinity for working with people with mental illness and/or co-occurring disorders.
• Must have the ability to work independently, have good problem solving skills and have the ability to work as part of a multi-disciplinary team.

Pay: $22.00 - $26.00 per hour
