JOB DESCRIPTION

Manager, Medical Case Management

Position Title Department Reports to Director of Programs (DOPS)

POSITION SUMMARY

Under the supervision of the Director of Programs (DOP), the Manager, Medical Case Management (MMCM) will be responsible for ensuring compliant, comprehensive case management services to persons living with HIV/AIDS within the boundaries of Charleston, Berkeley, and Dorchester counties.
This position will provide ongoing supervision, mentoring, training, and support to all Medical Case Managers (MCM) and must be proficient in all MCM essential duties, responsibilities, and MCM Ryan White Part B Standards

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

1. Assume day to day supervision of the case management process including training all new Medical Case Managers (MCM) and providing ongoing mentoring, training, and supervision of MCM staff; including PE documentation as needed.

2. Monitor MCM caseloads, productivity report and visit frequency for required Points-in-Care for open clients and meet with MCMs individually as needed.

3. Assume day to day supervision of any interns while working with the MCM staff.

4. Accept initial client referrals and complete Initial Contact/ New Intake form, register client in PE, brief eligibility screening and assign MCM.

5. Review and approve client requests at weekly service request meeting including HOPWA requests, dental and mental health referrals, and transportation requests
6. Responsible for entering HOPWA services into PE and providing the Office Manager an accurate weekly spreadsheet of approved services

7. Approve clients for MCM discharge and review clients potentially eligible for graduation at reassessment.

8. Oversee Graduation Program and maintain case load of graduated clients and in the peer program.

9. Review and approve all time off requests for MCMs.

10. Assist in serving as Duty Case Manager

11. Assist with transition of clients from Specialized Case Management to traditional MCM.

12. Oversee Truesdale Medical Center (TMC) and Palmetto Community Care (PCC) coordination by preparing TMC Care conference lists to ensure efficiency and in resolving issues to ensure client access to HIV medical care and ARV medications.

13. Meet with the Director of Programs & Services (DOPS) on a weekly basis to review MCM issues, concerns, and progress.

14. Complete 90 day and annual performance evaluations for all Medical Case Managers in conjunction with LMCM and DOPS.

15. Facilitate and lead the MCM weekly team meetings.

16. Provide progressive corrective action and/or Performance Improvement Plans (PIP) as needed in coordination with DOPS.

17. Responsible for working with and assisting DOPS including reconciling errors on reporting measures in PE.

18. Responsible for implementing new policy and procedure developments and training of MCMs including reviewing Ryan White B required service standards.

19. Attend internal and external meetings as needed and communicate information to MCM team- i.e. MCM Core workgroup, Care conferences, budget meeting, staff meetings.

20. Oversee MCMS additional responsibilities- dental vision, LID, TBRA, food bank

21. Serves as a member of the Continuous Quality Improvement Team

22. Other Duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Ability to adhere to the highest level of confidentiality and HIPAA guidelines.
  • Knowledge of HIV/AIDS issues and familiarity and comfort working with HIV positive persons.
  • Strong interpersonal, written, and oral communication skills, as well as problem solving.
  • Ability to work with people of diverse backgrounds with a variety of medical, socio-economical, and psychosocial issues.
  • Ability to interact well with people and provide an atmosphere of trust, mutual respect and caring.
  • Ability to manage stress appropriately, make decisions under pressure, handle multiple priorities, work alone, and manage others appropriately.
  • Strong and timely documentation skills including maintaining standards or confidentiality according to HIPAA regulations.
  • Proficiency with Microsoft Office Suite and databases systems.
  • Valid SC driver’s license and access to reliable transportation.

REQUIRED QUALIFICATIONS

  • Master’s Degree
  • Minimum of 2 years management experience.
  • Licensed Social Worker, Registered nurse, Nurse Practitioner, Physician Assistant, or Certified Substance Abuse Counselor preferred with two (2) years’ experience working in human services.
  • At least 2 years medical case management experience working with HIV+ clients preferred.

WORK ENVIRONMENT
While performing the duties of this job, the employee will normally work in a climate-controlled office environment. The noise level in the work environment is usually moderate.
