Aleca Home Health is a growing and busy Home Health agency providing high level residential healthcare to patients throughout Kitsap County. Aleca Home Health is a division of the Encore Communities/Sante’ Holdings, LLC family with our office located in Silverdale, Washington.

We take pride in being a Medicare & CHAP certified Home Health agency committed to providing “Care with Compassion” and fulfilling the needs of the community to the best of our ability by providing a variety of services to patients in the comfort of their residence. Our services include nursing, physical, occupational and speech therapy, social workers and home health aides. Specialized programming is available for those with COPD, pain, and congestive heart failure.

Job Summary

The Home Health Medical Social Worker (MSW) provides social work services to patients in their place of residence to assess the social, emotional and situational stress factors related to the patient's illness, need for care, response to treatment and adjustment to care. Services are provided according to the state MSW board rules, state and federal home health regulations, and the home health agency's policies and procedures.

Candidate Profile

  • A master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education or has a baccalaureate degree in social work, psychology, sociology or other field related to social work.
  • Current Washington state SWAL or SWICL
  • A minimum of 2 years MSW experience preferred.
  • Wellsky/Kinnser experience preferred.
  • Valid driver's license with proof of insurance in accordance with state requirements. 100% travel required.
  • COVID-19 vaccination required, or reasonable accommodation based on disability, medical condition, or religious belief that prevents vaccination.
