Full job description

JOB SUMMARY: Conducts comprehensive clinical assessments as a part of the diagnostic process, reviews ongoing delivery of services, identifies individual client needs and diagnoses substance use and/or mental health disorders. Conducts individual, family and group sessions to provide therapeutic treatment, and behavior modification using evidence-based practice to assist clients in reaching treatment goals. As a member of clinical staff and treatment teams, assists in case management and coordination of community resources to meet client needs.

STATUS: This position is exempt, full-time

Essential Functions:

· Demonstrate philosophical alignment with the agency’s values of person-centered, strengths based, individualized treatment

· Maintain family-centered services and support; Demonstrates understanding of importance of family, social determinants of health, and connection community that supports sustained recovery.

· Demonstrates understanding and awareness of cultural differences with ability to work with diverse populations

· Ability to work with wide range of individuals, including those with a criminal background

· Complete comprehensive mental health and SUD assessments and develop person centered, individualized services and support plan.

· Facilitate and effectively utilize evidence-based practices in parenting interventions, family, individual and group counseling settings.

· Provide education, coaching, and mentoring for new parents to support healthy attachment, reduce risks of adverse childhood experience, and support positive parenting techniques

· Maintain accurate records and complete all forms, assessments and file management records as directed

· Observe and comply with all OARs, CARF standards, HIPAA, and 42 CFR part 2

· Work in collaborative manner with all members of individual care team, including staff, community partners, families, and outside agencies.

· Cover clinical responsibilities for colleagues as required

· Participates in team meetings at the HOME and Dad’s program to support coordinated care for families

· Plan, develop, and implement goal oriented, therapeutic services for individuals and families

· Collaborate and communicate with staff and community treatment team members to provide holistic integrated treatment; act as primary counselor and case manager for assigned clients

· Demonstrates knowledge of and adherence to accepted ethical and behavioral standards of conduct and continuing professional development

· Participates in in-service training, clinical supervision, case staffing, consultations, and other meetings.

· Maintain accurate and timely records of service delivery in accordance with agency standards including evaluation of progress towards individually defined and measurable goals

JOB REQUIREMENTS

Experience and Skills Requirements:

· Ability to establish rapport with a wide range of clients and staff

· Experience working in substance use, behavioral health, early childhood, or school-based settings preferred.

· Excellent time management and organization skills

· Ability to work efficiently independently and collaboratively as part of a team.

· Effective oral and written communication skills

· Basic computer skills required i.e., Microsoft, Outlook, SharePoint etc.

Education/Licensure/Certification requirement:

· Current Registration as LPC, LCSW, Intern or registered QMHP within 30 days of hire

· Master’s degree in social work or other behavioral health related discipline eligible for licensure

· Licensure as an LPC, LMFT, or LCSW within three years is required for continued employment

· Ability to obtain CADC credentials within 18 months of hire

  • Approved background check, including Medicare exclusion list check
