Essential Function

Work with No Wrong Door (NWD) team to connect aging and disabled individuals and families to established support programs. Critical pathways providers to help facilitate access to needed home and community based supports and services, and streamline access to public benefits and application assistance. Also, provide Outreach working in conjunction with local NY Connects program for expansion and enhancement of NWD.

Principal Activities

Complete and report all required certifications and trainings, including Information and Assistance training, Person-Centered Assistance/Options Counseling, Benefits Advisement, the statewide database, resource directory, and participate in all necessary cross agency trainings.

Attend the Long Term Care Council (LTCC), NWD, and NY Connects staff meetings for the county they are located in.

Will be responsible for reporting all consumer activity and accurately document consumer services and required paperwork in the SPR database and the Consumer Service Record within two business days, as well as utilize technology for reporting, referrals, tracking and accessing resources, and coordinating three-way phone calls for a warm transfer.

Assist in identifying new resources to be included in the NY Connects LTSS Resource Directory and work in collaboration with specialized NWD/NY Connects standards to assist aging, people with disabilities and their caregivers.

Provide barrier-free access to services that accommodates people with special needs and access for individuals who speak languages other than English.

Provide information and assistance and the NWD screen for NY Connects contacts, including walk-ins and off site visits.

Facilitate a safe transition for consumers leaving a nursing home or institutional setting into the community by establishing partnerships with critical pathway providers.

Must have the ability to be mobile, work at various locations and deliver public education campaign developed and approved by the state.

Have an understanding or ability to learn resources in the delivery systems for aging and populations with disabilities, and how to connect consumers to more intensive services.

Have the ability to learn about Medicaid, Medicare, pooled trust and other financial resources that may be a barrier or needed to obtain services.

Complete applications and eligibility determination, as well as tracking outcomes on a monthly basis, or as required by NYSOFA.

Implement and adhere to the ACL’s National Standard on Options Counseling.

Perform any other duties as deemed legally and ethically necessary by the supervisor.

Job Qualifications

Bachelor’s degree in Human Services or Associate’s with work experience related to Human Services. Work experience can be substituted for educational requirements.

Knowledge of persons with disabilities, as well as federal and state laws pertaining to rights of persons with disabilities.

Excellent verbal and written communication skills.

Good organizational and analytical skills.

Desire to advocate for the rights of others.

Effectively manages multiple tasks and priorities

Accepts supervision and follows through with supervisor recommendations

Treats colleagues, staff, and program participants with respect

Maintain interest in helping others

Maintains a clean and safe working environment

Must be able to comply with HIPAA Requirements

Pay: $18.00 per hour

Expected hours: 35 per week
