Role Description

Faith Community Health (FCH) is seeking a highly motivated Nurse Practitioner to lead daily patient care and act as a clinical leader in our organization. The ideal candidate will be passionate about providing high-quality healthcare services to patients in a faith-based community setting. This position reports directly to the Executive Director of FCH and will work under the supervision of a collaborating physician who is contracted by FCH. The Nurse Practitioner will be responsible for representing FCH in the community and building strong relationships with patients and their families.

Responsibilities

  • Lead daily patient care, including physical examinations, medical assessments, diagnosis, and treatment of acute and chronic illnesses.
  • Work collaboratively with the collaborating physician and other healthcare professionals to develop and implement treatment plans for patients.
  • Act as a clinical leader in the organization, providing guidance and mentorship to other healthcare staff.
  • Represent FCH in the community, building strong relationships with patients, their families, and community leaders.
  • Participate in quality improvement initiatives and ensure that clinical protocols and standards are followed.
  • Maintain accurate and up-to-date medical records for all patients seen.
  • Keep abreast of current developments in healthcare and share knowledge with other staff.
  • Develop and implement health promotion and disease prevention programs for patients and the community.
  • Order diagnostic tests, such as lab work and imaging studies.
  • Prescribe medications and other treatments as appropriate for patient care.
  • Monitor and manage chronic health conditions, such as diabetes, hypertension, and asthma.
  • Provide patient education and counseling on healthy lifestyle choices, medication adherence, and disease management.
  • Coordinate care with other healthcare providers, including specialists and hospitals.
  • Assist with the recruitment, training, and supervision of other healthcare staff, such as medical assistants and nurses.
  • Ensure compliance with legal and ethical standards of healthcare practice, including HIPAA regulations and patient confidentiality.

Qualifications

  • Master's degree in nursing and certification as a Nurse Practitioner.
  • Active state license as a Nurse Practitioner and DEA registration.
  • Minimum of 3 years of clinical experience as a Nurse Practitioner in a primary care setting.
  • Ability to work collaboratively with other healthcare professionals.
  • Excellent interpersonal and communication skills.
  • Passion for providing high-quality healthcare services to underserved populations.
