We are searching for an energetic Occupational Therapist to join our team. Enjoy working one on one with your patients in the most functional environment possible --- their home. The ideal candidate will be patient care focused, organized, flexible, comfortable using a computer, and possess excellent communication & customer service skills.
Qualifications:
· Graduate of an approved occupational therapy curriculum.
· Possesses a current license to practice as an occupational therapist in Ohio.
Responsibilities:
· Evaluates and establishes goals based on the client’s current level of functioning and potential for improvement and performs re-evaluations as necessary.
· Guides and instructs the client in prescribed therapeutic, self-care and creative activities that are directed toward improving independence and physical and mental functioning.
· Establishes household management routines.
· Records and reports to the physician the client’s reaction to the therapy program or any changes in the client’s condition.
· Instructs the client and family in the use of prosthetic, orthotic and assistive devices.
· Instructs the family in the client’s total occupational therapy program.
· Prepares clinical/ progress notes of care services given and submits them to the Agency weekly.
· Participates in care conferences and other Agency meetings as necessary.
· Supervises the occupational therapy assistant.
· Participates with staff, client and physician in discharge activities and completes the occupational therapy discharge summary.
Organizational Relationship:
· Reports directly to the Director of Rehabilitation and Therapy.
Functional Abilities:
· Must be able to read 12 point type or larger.
· Must be able to communicate verbally and in writing in a manner understood by most persons.
· Must be able to stoop, bend and be able to lift and transfer clients.
· Must be able to travel to client’s residences.
Special requirements:
· Must have a reliable car, car insurance and a valid driver’s license.
· Must be able to successfully pass a background check.
Job Type: Full-time/Part-time/PRN
Compensation: $100,100.00 per year/$55 per point at 35 points a week (7 per day) plus incentive opportunities.
Sign-On Bonus: $6,000 for Full-time hires only.
