GENERAL DESCRIPTION:

The Outpatient Assessment Clinician is responsible for completing a full bio-psycho-social assessment, complete with a preliminary diagnosis and referral and linkage to treatment. Assessments are scheduled in advance and are offered in person or via telehealth for clients coming out of a hospital setting, residential unit, emergency department or calling of their own accord. Some follow-up is needed, such as notifying a referral source that a client did not show or assisting a client in linking with a recommended service/level of care. The Outpatient Assessment Clinician will function as a core member of the Outpatient Clinic and participate in weekly treatment meetings.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Collaborate & communicate with all staff to assure appropriateness and safety of programming for each client.

2. Assist with dangerous and potentially dangerous situations and take appropriate measures to assure client/worker safety.

3. Perform bio-psycho-social mental health and SUD assessments to provide a preliminary set of diagnoses. Assessments should include the following;

a. A review and summary of any referral information received,

b. The client’s reason for seeking treatment

c. Lethality screening,

d. A detailed list of the client’s current problems and symptoms

e. A detailed list of any current or historic substance use history

f. Medication history and physical health history

g. The client’s mental health history – including documentation of any hospitalizations or outpatient visits.

h. The client’s family and social dynamic. and refer clients to community programs upon discharge. Assessments include bio-psychosocial, psychiatric, and alcohol/drug information using the Netcare Assessment tools.

i. Legal, employment, financial, and/or veteran histories and current statuses

4. Assess for risk for all clients receiving assessment services and consultation as appropriate.

5. Make appropriate recommendations for level of care using the CSSR-S, ODADAS Clinical Protocols, DSM criteria and Netcare protocols.

6. Provide general case management services for clients using the follow-up process.

7. Make appropriate internal/external emergency and crisis intervention referrals; dispatches mobile crisis services, refers to CSU and Miles House as appropriate.

8. Obtain sources of collateral information from family members, therapists, case managers, family doctors, etc. as appropriate.

9. Perform linkage of clients to other mental health agencies. Perform linkage to Netcare’s outpatient clinic as appropriate.

10. Complete all required quality data scales, such as the OQ, and submit data within indicated timeframes.

11. Serve as multi-disciplinary team member on the outpatient treatment team. Come prepared to treatment team with information regarding clients referred to the clinic.

12. Be a part of the outpatient team and assist in daily Clinic needs like answering the phone, the door, and faxing information and other duties as needed.

13. Performs related duties as required.

14. Assures adequate, accurate and timely documentation.

ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES

1. Ability to relate to clients with a variety of age, sex, economic, educational and professional backgrounds.

2. Ability to apply and integrate theory, knowledge, values and practices of social work and/or counseling profession.

3. Ability to engage in problem solving processes.

4. Ability to understand, articulate and apply the dynamics of substance use disorders.

5. Demonstrated skills in the use of personal computer to record client data, written documentation of client assessment and interventions, correspondence or reports.

6. Ability to interpret and assess data.

7. Working knowledge of the DSM-V.

MINIMUM QUALIFICATIONS:

Master’s or Bachelor’s LSW/LPC; LISW/LPCC. Experience completing diagnostic assessments preferred.
