The schedule for this position will include 2:45pm-10:45pm shifts.

ESSENTIAL JOB FUNCTIONS:

Assists residents with daily personal routines including, but not limited to personal hygiene, bathing, dressing, grooming and toileting reminders in accordance to the Individual Service Plan (ISP).

Establishes a compassionate environment by providing emotional, psychological and spiritual support to residents, friends and families.

Observes residents to identify changes in behavior and/or physical condition or appearance and reports any concerns or needs to Director/Case Manager, and notes changes in daily report book.

Ensuring resident rooms are safe and well-organized; and assist with changing linens and making beds as needed.

Assists in serving meals and monitoring dining room during scheduled meal times.

Encourage proper food and fluid consumption while respecting resident rights and choice; records and reports changes in resident’s eating habits.

Ensuring residents are taking prescribed medications in accordance to physician orders and as defined in medication procedure; and sends out refill requests/delivers medicine to residents.

Encourage residents to socialize and participate in resident leisure activities and scheduled activity programs.

Providing companionship and fostering trusting relationships.

Escort residents to appointments as needed.

Performs hourly safety rounds of building (i.e. lounges, hallways, basement, etc.) to ensure that all is secure, monitors emergency call systems, and knows the location of each resident. Remind residents as needed to sign out when leaving the building.

Maintaining resident’s records in a timely manner and in accordance with facility policies and procedures.

Maintains a safe and clean living and working environment by adhering to facility policies and procedures, including infection control and emergency preparedness.

Follows proper procedures in emergency situations and responds promptly and positively to resident requests for assistance.

Participates in care conference to discuss resident’s needs and concerns.

OTHER JOB FUNCTIONS:

Encourages teamwork fostering a spirit of cooperation among team members, communicating information, building rapport, participating in team problem solving methods.

Actively participates as a member of the department in improving quality of care/service, and completes quality assurance audits.

Responds accordingly to procedures for fire drills and disaster drills.

Utilizes products, supplies and equipment appropriately.

Attends mandatory education programs on fire, safety, infection control, first aid, etc. annually, and completes monthly inservice programs.

Attends education/training programs, which will enhance/increase knowledge and skills relative to performance expectations.

Performs other related duties as assigned.

JOB QUALIFICATIONS:

High School Education or equivalent.

Previous health-related experience is preferred.

Basic understanding and a genuine interest in the care of the geriatric resident is required.

Excellent communication, both written and verbal is required.

Ability to work in a team environment is required.

The base range for this position is $16.80 - $17.30 per hour commensurate with work location, experience, skills, certifications, education & prior accomplishments.

The position may be eligible for additional compensation not listed here, including but not limited to shift differentials, incentives or bonuses, as applicable.
