Location: Hampton Roads, Virginia (Newport News, Hampton, Richmond, Chester, Poquoson)

Position Type: Full-Time

Salary: $41,000 – $50,000 per year

Position Overview

The Program Coordinator is responsible for overseeing the development, implementation, and coordination of Individual Support Plans (ISPs), Behavioral and Therapeutic Support Plans for individuals within the Sponsored Residential program. This role ensures compliance with DBHDS, DMAS, and HCBS regulations, and involves direct interaction with individuals, families, and community partners to deliver person-centered services.

Key Responsibilities

  • Person-Centered Planning & ISP Development: Lead the creation and ongoing implementation of ISPs and behavioral support plans, ensuring they are tailored to each individual's needs and preferences.
  • Regulatory Compliance: Ensure all services adhere to DBHDS, DMAS, and HCBS regulations, maintaining up-to-date knowledge of relevant policies and procedures.
  • Quality Assurance: Conduct regular reviews of service delivery, documentation, and outcomes to ensure high-quality care and continuous improvement.
  • Staff Supervision & Training: Provide guidance and support to Sponsored Residential Providers, ensuring they receive necessary training and maintain certifications in CPR/First Aid, Medication Administration, and other required areas.
  • Community Integration: Facilitate opportunities for individuals to engage in community activities, promoting normalization and inclusion.
  • Documentation & Reporting: Maintain accurate records of services provided, incidents, and progress notes, ensuring compliance with all documentation standards.
  • Collaboration: Work closely with families, guardians, Community Services Boards (CSBs), and other stakeholders to coordinate services and support individuals in achieving their goals.

Qualifications

  • Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field or 5 to 10 years’ experience managing a minimum caseload of 20 or more individuals.
  • Experience: Minimum of 1 year of experience in a supervisory or program coordination role within a behavioral health or developmental services setting.
  • Knowledge: Strong understanding of DBHDS, DMAS, and HCBS regulations and service delivery models.
  • Skills: Excellent communication, organizational, and leadership skills. Proficiency in Microsoft Office Suite and electronic health record systems.
  • Licensure: Valid driver’s license and reliable transportation.

Preferred Qualifications

  • Experience with Waiver Management System (WaMS) and Computerized Human Rights Information System (CHRIS).
  • Certification in Medication Administration.
  • Experience in quality assurance and compliance monitoring.

Work Schedule

  • Hours: Monday – Thursday, 8:00 AM – 5:00 PM; Friday, 8:00 AM – 3:30 PM
  • On-Call: Availability for 24-hour on-call support as needed.
