Responsibilities
· Receive and process referrals
· Schedule and conduct intake assessments to screen clients in and/or out of the program
· Conduct the matching process with clients and providers/volunteers
· Provide on-going support for providers and volunteers
· Facilitate transitions for clients and providers
· Attend team meetings
· Collaborate with providers, case workers, and/or guardians/clients to staff client concerns/work towards goals
· Advocate and make recommendations for service and goal changes
· Communicate with case workers about services
· Schedule and facilitate supervision and consultation meetings
· Document meetings, update reports, and file necessary paperwork
· Consult during crisis/emergency situations
· Facilitate extra expense requests/activity approval, releases of information, incident reports, and field-trip form completion and return
· Attend staff meetings and trainings
· Conduct orientations and train new providers and volunteers on policies, procedures, and documentation
· Collaborate to develop ongoing training and support
· Create and distribute ongoing tools to ensure providers and volunteers are supported and successfully managing their own caseloads
· Initiate and assist in coaching process
· Conduct Quality Assurance tasks by tracking and reviewing documentation for appropriate content, correct spelling, grammar, punctuation, format, accuracy, and adherence to company guidelines
· Procure and prepare meals for Workshops groups
· Other tasks as assigned
Qualifications
Must be at least 18 years of age
High School diploma or equivalent
Appointment will be conditional upon successful completion of criminal and caregiver background checks

Pay: From $18.00 per hour
Expected hours: 20 – 25 per week
