PROGRAM MANAGER: Food As Medicine Programs– Job Description
ORGANIZATION SUMMARY:
Western New York Integrated Care Collaborative (WNYICC) is an innovative non-profit corporation, incorporated in 2016. As the Community Care Hub of a multi-county Social Care Network of Community-Based Organizations (CBOs) and government agencies, we are strategically positioned to respond to and capitalize on opportunities to advance community-based integrated health care and to bring sustainability to community-based programs and services which address social determinants of health. WNYICC’s role in the network is to provide administrative contracting, program coordination, billing, technical support, training, and quality assurance on a regional level. The WNYICC Network works collaboratively to navigate the changing healthcare landscape together to improve health outcomes in our Western New York community.
POSITION SUMMARY:
The Program Manager: Food As Medicine is responsible for managing WNYICC's Food As Medicine Programs under the direction of Director of Nutrition and Wellness and Sr. Director of Operations and is valued member on the Operations Team.
- This is a hybrid position which will include both field-based and office-based responsibilities. Work From Home / Remote office is an option for office-based responsibilities. Travel may include local, regional, and state-wide for in-person responsibilities and out-of-state for conferences.
RESPONSIBILITIES
- Manage Food As Medicine Programs including Medical Nutrition Therapy, diabetes programs, and food programs
- Provide trainings, chart reviews, program coordination, reporting, quality assurance, credentialing, marketing, and promotion
- May Include: On-site visits, menu development or analysis
- Provide nutrition counseling; develop of education materials, group presentations
- Developing new programs and processes with Management Team
- Provides support for assigned Committees and / Workgroups of the Network.
- Professionally represents WNYICC on network-wide initiatives.
- Demonstrates excellent customer service on phone, in-person, via email and in all aspects of employment at WNYICC.
- Communicates in a positive manner both internally and externally to partners.
- Assists Team in a wide array of responsibilities from event planning to reports.

QUALIFICATIONS:
- Requires a minimum of a bachelor’s degree in nutrition or dietetics (master’s preferred)
- Requires New York State Certified Dietitian-Nutritionist (CDN) certification and must be a Registered Dietitian through the Commission on Dietetic Registration (CDR) (or eligible to apply)
- Demonstrated experience working with clients who have pre-diabetes and/or diabetes. Certified Diabetes Care and Education Specialist, preferred.
- Demonstrated experience providing Medical Nutrition Therapy.
- Familiarity with Food As Medicine principles.
- Strong computer/technology skills including: electronic health record, Microsoft Office-Suite, Outlook, and more.
- Multi/Bi-lingual is a plus.
- Ability to relate effectively to diverse groups of people from all social and economic segments of the community with a track record of building authentic, constructive relationships with others.
- Team player who enjoys working both collaboratively and independently.
- Must have a reliable form of transportation, as a significant portion of the programs will be conducted in the field throughout the Western and Central New York regions. Must provide proof of auto insurance if using own vehicle.
