Job Description

The Population Health Project Coordinators primary purpose is creating, managing and tracking clinical programs and projects. The Population Health Project Coordinator will help advance STPCHC’s key clinical and performance objectives and measures. This role also facilitates the development of internal and external reports and delivery of data to support and monitor the action plans to improve customer experience, quality of care and patient outcomes, and address patient social needs.

Essential Duties & Responsibilities

· Develop clinical programs for specific patient populations

· Organize and facilitate all project meetings

· Ensure timely completion of all project benchmarks and deliverables

· Design work plans, workflows and processes by which these programs would be managed

· Communicate and help develop necessary policies and procedures to aide in execution

· Facilitate and track program pilots as deemed necessary

· Design and analyze performance and outcomes reporting for clinical programs

· Track progress of action plans, present outcomes to peers, the leadership and outside partners

· Monitor program integrity and efficacy through routine quality improvement processes and report findings to key stakeholders

· Responsible for additional population health management tasks and initiatives defined by management

· Bachelor’s degree in Public Health, Health Promotion, Psychology, Human Development, or related field

· A combination of school and experience will be considered in lieu of education


· Minimum two years of Project Management experience.

· Previous healthcare experience a plus

Knowledge, Skills & Abilities

· Understanding of clinical programs and population health initiatives such as HEDIS and UDS

· Proficient with Microsoft Office Suite skills including Excel, Word, and PowerPoint.

· Excellent written and verbal communication skills.

· Ability to manage multiple projects and tasks simultaneously.

· Ability to set goals and judge results in accordance with the highest standards; care with sensitivity and respect; promote teamwork to achieve STPCHC mission;

· Ability to think critically, work independently, manage multiple assignments, compose relevant written presentations; work well under pressure, problem solve, organize and prioritize workload, recall information with accuracy, be attentive to detail.

· Ability to relate and communicate positively, effectively, and professionally with others, provide leadership, be assertive and consistent in enforcing policies, work calmly and respond courteously when under pressure, lead, supervise, teach, and collaborate and accept direction.

Bilingual (English/Spanish) with comprehension of medical terms in both Spanish and English are required.

COVID-19 Considerations

· All STPCHC employees must be fully vaccinated against COVID-19 as a condition of employment. Successful candidates for this position will be required to submit proof of vaccination against COVID-19 or request an exemption for qualifying medical or religious reasons during the onboarding process. Candidates should not present proof of vaccination until instructed to do so by the hiring department. All STPCHC employees are required to wear masks in the office irrespective of vaccination status.
