WeCareDMV Day Services, Inc., formerly United Cerebral Palsy of Washington, DC & No. VA, Inc., is a non-profit organization that operates a day program for adults with developmental disabilities in the District of Columbia. We are seeking a full-time QIDP (Qualified Intellectual Disabilities Professional) / Case Manager to provide case management services for an assigned caseload, provide oversight to a team of direct support professionals, and monitor documentation and services recorded in the electronic records system. This position is responsible for completing all duties in accordance with UCP’s policies and procedures in coordination with Medicaid and the Department on Disability Services (DDS) directives. Monday through Friday work schedule, with tentative hours of 9:00 am to 3:30 pm.

The QIDP/Case Manager will have responsibility towards clinical and problematic issues for their assigned caseload and will take a lead advocacy role to ensure individuals receive appropriate care, services, equipment and successful community integration. The QIDP/Case Manager will be required to monitor documentation, assist with goal and activity development, respond to issues on the MCIS dashboard, write weekly/monthly notes and quarterly reviews, complete incident reports and participate in the development and implementation of individual support and behavioral plans. The QIDP/Case Manager prepares a variety of reports and attends court hearings and internal/external interdisciplinary team meetings. This position creates and analyzes data to provide recommendations for improvement. This position is also responsible for overseeing the financial accounts of the individuals supported.

The QIDP/Case Manager supervises the work performance of an assigned staff of DSPs and counsels, guides and coaches as it is needed. The QIDP/Case Manager will also have responsibility for providing regular reviews, audits, and follow-up of information contained in the iCare Manager system.

This position requires good interpersonal skills, a desire to help people and a passion for the people we support, the ability to develop positive relationships and effective verbal and written communication skills. Must be extremely detail-oriented, organized, possess problem solving skills and ability to multi-task and think creatively. Requires strong computer skills with proficiency in Microsoft programs (Word, Outlook, Excel).

Candidate must have a Bachelor's Degree in a Human Services or related field, a minimum of two (2) years of experience working with individuals with intellectual or developmental disabilities with demonstrated ability to fulfill job responsibilities, and one (1) year of experience in providing supervision and developing individualized program plans and/or coordinating services. Must possess a valid driver's license and good driving record and pass a criminal background check.
