Position Title
Residential Coordinator
Position Pay Level
$57,000 annual salary plus a $3,000 annual bonus opportunity for a $60,000 total annual compensation opportunity.

Position Summary
The Residential Coordinator serves as part of the HighPointe Residential Services Team and reports directly to the Residential Director. As a Coordinator, you will be responsible for overseeing and having direct contact daily with clients, providers, and family caregivers to meet all compliance requirements.
The coordinator position is responsible for monitoring quality of services and adhering to all organizational policies and procedures to meet State regulatory guidelines. As a Coordinator you will work with a variety of clients, providers, and families, building and fostering a professional relationship through truth, honesty, and quality of care, which are the foundations of High Pointe’s values and our mission for always providing the highest quality of care.
This is hybrid role working from home and from our Fort Collins program facility.
Responsibilities and Duties:
- Work closely with the Residential Director to deliver excellent client and provider support.
- Communicate and implement strategic goals and tasks associated with client and provider supported care in residential settings.
- Support clients and providers with problem-solving and decision-making as independently as possible following all operational policy and procedures.
- Communicate effectively with host home providers, family caregivers, HighPointe Compliance Director and all internal departmental managers to ensure adherence to policy and procedural guidelines.
- Conduct on-site provider and family caregiver home visits monthly and ensure all residential standards comply with regulatory requirements.
- Create and maintain a healthy relationship with all clients and providers to ensure continuity of care and respect for client communication and choice in all settings.
- Participate in annual service plan, interdisciplinary team, and medical monitoring meetings and submit appropriate documentation as required and correct deficiencies timely.
- Monitor completion of required documentation to ensure timely submission to support strategic goals, along with adherence to HighPointe policies and Colorado state compliance standards.
- Work with the Residential Nursing team to ensure appropriate medical administration and documentation is accurate and in compliance with state standards.
- Conduct and report scheduled and random on-site evaluations and surveys.
- Oversee personal property inventory process to ensure proper completion.
- Maintain all client and provider master files.
- Ensure timely and professional follow-up and response to provider requests
- Assist in scheduling and promoting required education and training sessions.
- Participate in required training and recertification training.
Qualifications
- Bachelor’s degree in Social Services, Rehabilitation Services or related field.
- 5 years’ minimum experience working within the disability community. Experience within the I/DD community highly preferred.
- Detailed understanding of I/DD and Colorado residential support and waiver services.
- 24-hour on-call position. Requires emergency response to residential needs.
- Must have a current Colorado Driver’s License, Colorado-registered vehicle and Colorado automobile insurance meeting minimum state requirements.
Skills and Abilities
- People oriented and very familiar with collaborative teamwork.
- Competency in Microsoft Office applications and ability to learn new software programs - as necessary - for timely completion of projects.
- Excellent oral, written, technical, interpersonal and organizational skills.
- Ability to work at a fast pace, within an ever-changing work environment.

- Ability to maintain confidentiality and to effectively communicate with directors, managers and providers, as well as clients and families.
- Ability to maintain professional communication, as related to email, phone calls, written text and letter communications, project and hour tracking, etc.
- Ability to identify barriers to tasks completion and propose realistic and professional solutions to Residential Director.
