Job Description

The Population Health Resource Care Coordinators primary purpose is to develop and organize comprehensive resources that allows the health center to better care for individual and population health with an emphasize on social determinants of health and health disparities.

Essential Duties & Responsibilities

Under the supervision of the Population Health Project Coordinator, the Population Health Resource Care Coordinator will:

· Develop and maintain a resources database that serves to meet patient health and non-medical needs. This includes but is not limited to health screening tools, social determinants of health screening tools and resources, specialty referral resources, educational health materials, and local, state and federal health laws and regulations

· Design work plans, workflows and processes by which these resource can be accessible to the health center workers and patients

· Working closely with STPCHC leadership, physicians and care teams to ensure that resources are appropriate and accessible

· Outreach local organizations, participate in health and resource fairs, coalition meetings, and other events in the community with the goal of expanding the resource database

· Responsible for additional population health management tasks and initiatives defined by management

· Bachelor’s degree in Public Health, Health Promotion, Psychology, Human Development, or related field

· High School Diploma or GED


· A minimum of two years of experience working in a healthcare setting.

· Working with the underserved patient population is highly preferred.

Knowledge, Skills & Abilities

· Understanding of clinical programs and population health management.

· Proficient with Microsoft Office Suite skills including Excel, Word, and PowerPoint.

· Excellent written and verbal communication skills.

· Bilingual proficiency, English and Spanish speaking.

· Ability to manage multiple projects and tasks simultaneously.

· Ability to think critically, work independently, manage multiple assignments.

· Ability to relate and communicate positively, effectively, and professionally with others, provide leadership, be assertive and consistent in enforcing policies, work calmly and respond courteously when under pressure, lead, supervise, teach, and collaborate and accept direction.

Working Conditions

While performing the duties of this job, the employee is typically required to regularly conduct work in a controlled office environment, moving around the office as needed, sitting at a desk, using office equipment, including but not limited to a personal computer, fax machine, copier and telephone, read, write, listen, speak, and understand English, have the ability to work independently, be dependable and maintain confidential information, integrity and composure at all times.

Constant contact with individuals internal and external to the organization occurs via various methods of communication, typically including face-to-face discussions, electronic mail, phone calls, written letters and/or faxes.

COVID-19 Considerations

· All STPCHC employees must be fully vaccinated against COVID-19 as a condition of employment. Successful candidates for this position will be required to submit proof of vaccination against COVID-19 or request an exemption for qualifying medical or religious reasons during the onboarding process. Candidates should not present proof of vaccination until instructed to do so by the hiring department. All STPCHC employees are required to wear masks in the office irrespective of vaccination status.
