FULL TIME- 35 HRS PER WEEK - 8:30-4:30 M-F

NO NIGHTS AND WEEKENDS!!!!! 13 PAID HOLIDAYS!!!!!

UNION POSITION- $21.21 per hour (non-negotiable)

· Provides home visits to pregnant women & infants residing in Kankakee County for the purpose of needs assessment, education, and case management.

· Provides case management services to pregnant women and infants.

· Provides education to clients for healthy pregnancy and infant growth and development.

· Maintains appropriate documentation of assessment, actions, client needs and referrals.

· Maintains accountability of case management services.

· Maintains close working relationships with clients’ assigned RN Case Managers.

· Performs outreach efforts as directed to assist with maintenance of local area maternal child health provider network.

· Performs appropriate emergency response duties as assigned according to agency needs.

· Performs other duties as assigned.

Is professional, mature, and self-controlled. Maintains strict confidentiality of client and office information. Displays organizational skills and ability to work flexibly and independently.

Education and Training

1. Master’s Clinical Social Worker licensed pursuant to Section 9 or Social Worker licensed pursuant to Section 9A of the Clinical Social Work and Social Work Practice Act (IL Rev.Stat. 1991, ch.111, par.6351 et seq.) [225ILCS 20] and 68 IL Adm. Code 1470 AND

a. One year of experience in providing services to families with young children OR.

b. Supervision by RN, licensed social worker or licensed clinical social worker with length of experience required in subsection ©(1)(B)(i) of IL MCH Admin Code tit.77 630.22 OR

2. Masters or Baccalaureate degree in a behavioral science, social science, or health-related area; or a baccalaureate degree in any other area AND

a. One year experience in child, family, or community services OR

b. Associate degree and two years’ experience in child, family, or community services.

c. Case Managers meeting only this qualification must be supervised by a case manager meeting requirements of subsection ©(1)(A) or (c)(1)(B) of IL MCH Admin Code tit.77 630.22 until they have a total of two years of supervised case management experience

Job Requirements

Requires driver’s license and own vehicle for use in job duties, as well as current car insurance. Requires mobility to sufficiently get in and out of vehicle to perform home visits and ability to lift 15 lbs. Requires knowledge and efficiency with computers and data entry. Requires vision adequate to read and analyze written and printed documents. Requires hearing and speech skills adequate to answer telephone calls and to communicate with clients and staff. Requires ability to follow verbal and written directions. Requires ability to respect and adapt readily to a variety of client/staff situations with maturity and emotional stability.
