Full job description
We are seeking a Speech Language Pathologist (SLP) to evaluate and treat residents with swallowing disorders, communication impairments and cognitive deficits in a skilled nursing and short-term rehab setting. The SLP will work closely with the interdisciplinary team to promote safety, independence and quality of life.
Responsibilities
- Implementation of evidence based practices for the assessment and treatment of swallowing, communication and cognitive deficits
- Development and implementation of individualized treatment plans
- Participation in staff and family education and training
- Participation in interdisciplinary team meetings and care planning
- Understanding of Patent-Driven Payment Model and how it impacts therapy
- Maintain effective, accurate and timely documentation in accordance to facility, state and federal guidelines
- Demonstrate professional, ethical and regulatory standards as well as compliance with HIPPA and facility policies
Qualifications
- Masters degree in Speech Language Pathology
- Current NYS SLP license; CCC SLP preferred
- SNF and rehab experience
- Strong communication, clinical and documentation skills
