Full job description

We are seeking a Speech Language Pathologist (SLP) to evaluate and treat residents with swallowing disorders, communication impairments and cognitive deficits in a skilled nursing and short-term rehab setting. The SLP will work closely with the interdisciplinary team to promote safety, independence and quality of life.

Responsibilities

  • Implementation of evidence based practices for the assessment and treatment of swallowing, communication and cognitive deficits
  • Development and implementation of individualized treatment plans
  • Participation in staff and family education and training
  • Participation in interdisciplinary team meetings and care planning
  • Understanding of Patent-Driven Payment Model and how it impacts therapy
  • Maintain effective, accurate and timely documentation in accordance to facility, state and federal guidelines
  • Demonstrate professional, ethical and regulatory standards as well as compliance with HIPPA and facility policies

Qualifications

  • Masters degree in Speech Language Pathology
  • Current NYS SLP license; CCC SLP preferred
  • SNF and rehab experience
  • Strong communication, clinical and documentation skills

May include holiday rotations

Job Type: Part-time

Pay: $41.00 - $46.50 per hour

Expected hours: No less than 15 per week
