Full job description

We are seeking a Speech Language Pathologist (SLP) to evaluate and treat residents with swallowing disorders, communication impairments and cognitive deficits in a skilled nursing and short-term rehab setting. The SLP will work closely with the interdisciplinary team to promote safety, independence and quality of life.

Responsibilities

Implementation of evidence based practices for the assessment and treatment of swallowing, communication and cognitive deficits
Development and implementation of individualized treatment plans
Participation in staff and family education and training
Participation in interdisciplinary team meetings and care planning
Understanding of Patent-Driven Payment Model and how it impacts therapy
Maintain effective, accurate and timely documentation in accordance to facility, state and federal guidelines
Demonstrate professional, ethical and regulatory standards as well as compliance with HIPPA and facility policies
