In 2012, Brooklyn Aids Task Force changed its name to BAC and successfully made the transition from an AIDS-centered organization to a multi-service community-based healthcare organization. Brooklyn Aids Task Force was the very first HIV/AIDS organization in Brooklyn prioritizing serving persons living with HIV/AIDS (PLWHA) since 1986. Today, BAC provides mental health and substance use-related social services, comprehensive HIV/AIDS care, and coordinated health care and supportive services to under-served racial and ethnic minority communities in the Brooklyn and other parts of New York City.

Job Summary:

BAC has TEMP, Full-time job opening for Case Managers supportive case management services to clients living in OASAS MRT Scatter-site Housing.

(BAC) is a nonprofit organization that provides a range of healthcare services to underprivileged communities in Brooklyn and other parts of New York City.

** This specific assignment is expected to end in October 2024. However, this candidate may have the option to move to other permanent Case/Care Manager positions within the agency. **

Education and Work Experience Requirements:

- Minimum HS Diploma or equivalent with 2 years of field experience providing housing and/or case management services to homeless populations, people with substance use disorders, and/or people with psychiatric illness.

- Minimum Bachelor’s degree in social work, Human Services, or related field, plus 1 year of experience providing housing and/or case management services to the preferred populations."

Duties and Responsibilities:

- Conduct intake assessments, quarterly re-assessments, monthly (at minimum) home visits, and develop and implement initial, annual, and quarterly service plans for each client on a caseload of 20-25.

- Complete initial, annual, and updated apartment inspections.

- Coordinate move-in for new clients and relocation for existing clients when necessary.

- Assist clients with establishing utility services and monitor monthly payments to ensure utilities are not disconnected for non-payment.

- Calculate, collect, and track monthly rent payments from each client to ensure they make appropriate monthly rent payments.

- Resolve all apartment repair/maintenance needs via BAC maintenance staff and advocacy with the clients’ superintendents, building managers, and landlords.

- Conduct bi-annual case conferences with collateral providers.

- Provide referrals for various services as necessary.

- Facilitate and co-facilitate tenants’ meetings and Independent Living Skills groups.

- Complete entry, annual, and exit HMIS assessments.

- Compile and maintain a resource directory including, but not limited to, landlords and service providers; maintain positive working relationships with collateral contacts.

- Perform other related duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

· Have a deep understanding of substance use, recovery, and mental health issues.

· Possess exceptional computer skills, particularly in Microsoft Office (Word, Teams, Excel), and I can quickly adapt to new programs.

· Well-versed in electronic medical records (EMR) and have strong writing and communication abilities.

Job Types: Full-time, Temporary

Pay: $26.07 per hour

Expected hours: No less than 35 per week
