JOB DESCRIPTION

GENERAL RESPONSIBILITIES:

The Targeted Case Manager will be responsible for ensuring that person served has the maximum independence and successful inclusion into the community living though medical, social, educational, and other needed services.

Targeted Case Management includes and or all of the following services: Assessment, Development of specific support/care plan, Referral and related activities, Monitoring and Follow up

ESSENTIAL FUNCTIONS:

75% BILLABLE ACTIVITY

● Advocate and promote the personal choice of person served.

● Develop, and keep updated Person Centered Plans on each person served on caseload.

● Assure follow through and monitoring of Behavior Plans are completed.

● Promote the participation of person served on an ongoing basis in discussion of plans, goals, and services.

● Facilitate and monitor caseload schedule of programs and services outlined in the person-centered plan.

● Assure the person centered plan meets applicable organization, state, and regulatory standards.

● Maintain regular and ongoing communication with parents, guardians, staff and others concerning person served.

● Coordinate transportation when needed for the person, may require some transporting of the person, when no supports are available. This will be reviewed with supervisor on a case-by-case situation.

● Facilitate discharge and transfer process and make arrangements for follow up and supportive services.

● Coordinates evaluations and assessments to determine strengths, needs, and preferences of person served, as appropriate.

● Assure the rights and dignity of person served.

25% ADMINISTRATIVE

● Maintain ongoing and regular documentation for caseload

● Collect, disseminate, and record information and data concerning caseload in a timely and accurate manner.

● Maintain confidentiality of persons served.

● Participate in the Quality assurance reviews as assigned to the agency.

● Report and properly document accidents, illnesses, and /or incidents, including suspected cases of abuse, neglect and exploitation.

● Assist person served and parents/guardians in securing resources, as appropriate.

● Serve as liaison for parents, guardians, and community resources, as needed.

● Develop and recommend strategies to meet service needs for persons served within the organization or community programs and served as needed.

● Behavior Management Practices; Assure that approved behavior management techniques comply with organization policy and procedures.

● Maintain training hours as required by licensing regulations.

● Coordinate or provide technical assistance and follow up to training staff on issues pertinent to person centered plan.

Education Requirements:

● Bachelor's Degree in Human Services or related field.

● SUBSTITUTION FOR EDUCATION: Successful work experience in the related field to equal 1080hrs per semester

Experience Requirements:

● Minimum of six months experience working with people with developmental disabilities

● A valid driver’s license and adequate transportation

Physical Requirements:

Prolonged periods sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times.

Job Type: Full-time

Pay: $35,000.00 - $49,900.00 per year
