Company: Better Together ND

Position: Transition Coordinator $23.00- $25.00 per hour

Location: Fargo, ND

Job Summary:

  • Assists individuals with planning and coordination of the activities necessary for a successful transition to community living and successful community adjustment.
  • Coordinating the transition planning process with the individual/family and all team members.
  • Assuring ongoing communication and coordination with everyone involved throughout the process from the day of referral until the end of the individual’s program eligibility.
  • Transition Coordinators will provide follow-up contacts after transition to the community, based on the adjustment needs/preferences of the individual.
  • The types of visits will be determined by the individual, the Transition Coordinator, and their team with a minimum of monthly contact during their participation in the service.

Responsibilities:

  • Explain ADRL Transition or Diversion Services to the individual
  • Secure consent to participate in ADRL Transition or Diversion Services
  • Schedule and Coordinate team meetings with team members
  • Coordinate or complete steps that lead to transition/diversion
  • Identify spending needed for transition or diversion
  • Request approval for spending from ADRL Referral Specialist
  • Make necessary purchases
  • Assure all team members are aware of their responsibilities in the planning process.
  • Assist the individual in the home day of transition and at least monthly.
  • Complete Case Notes in THERAP such as the Referral, Plan, Termination notes
  • Provide Case Closure Notice with term of eligibly has ended.
  • Coordinators are required to complete the care coordination training provided by the State of North Dakota after being hired.
  • Valid Driver’s License
  • Must successfully complete a background check.

Requirements:
- Bachelor's degree in a relevant field or have four (4) years of experience in a human service setting providing direct services to individuals.
- Proven experience in program development and management.
- Strong budgeting skills to effectively allocate resources for service transitions.
- Excellent administrative abilities to oversee documentation and reporting processes.
- Proficient in relationship management to foster collaboration among teams and stakeholders.
- Experience in managing projects from initiation to completion.
- Knowledge of behavioral health or social work practices is a plus.
- Ability to work with individuals with developmental disabilities is advantageous.

Preferred Skills:
- Prior experience in service transition management is preferred but not mandatory.
- Exceptional written and verbal communication skills.
- Self-motivated individual with a proactive approach to problem-solving.
- Familiarity with working in a fast-paced environment such as program management.
