Care Liaison position:

We are seeking an individual for a part-time role, guaranteed 20 hours a week and upwards of 24 hours a week, for our Care Liaison position. Individuals with a strong background in human services, such as Psychology, rehab, social work, or direct client care would be preferred for this role. The role provides care coordination, emotional support, social interactions, and skill building to our clients living with disabilities.

**Driving clients is a key aspect of this role, but there will not be more that 2 or 3 driving appointments per week. You would use your own vehicle, but mileage is reimbursed.**

This is a hybrid role, which requires time in our office in Wayne, PA but also allows for some work from home after a 3-month introductory period.

This role is flexible in nature and may not hold a set schedule every week.

Key duties:

Care Management:

  • Provide coverage for routine appointments with clients, such as grocery shopping, doctors appointments, activities/social outings
  • Facilitate client participation for group events and socialization opportunities
  • Provide logistical support for record keeping and benefits attainment including procurement of releases of information, client identification documentation, bank statements, etc.
  • Schedule client appointments including doctors appointments, assessments, and care planning
  • Write Annual Care Plans for unassigned Care Management clients
  • Support for overflow of routine redeterminations such as ACP, Rent Rebate, CAP and SNAP

Trust Administrative Support:

  • Ordering and purchasing approved items
  • Assisting clients in obtaining documentation needed for requests
  • Procuring services for clients that the Trust intends to pay for
  • Provide support/coverage to the Trust Annual Review process
  • Cover Trust phone lines as needed
  • Assist with bill pay logistics
  • Scan files

Job Type: Part-time

Pay: $20.00 - $22.00 per hour

Expected hours: 20 – 24 per week
