Seeking a full time LPNs to provide continuous end-of-life care to hospice patients in our service areas. Under the supervision of RN, the LPN will provide routine patient visits, wound care, continuous care services, in-home respite care, personal care for patients and On-Call coverage when needed.

Qualifications:

  • LPN licensed in NY Sate
  • 2+ years of LPN experience
  • Reliable motor vehicle required in order to reach patients on our program OR open to commuting with mass transit.
  • Ability to work with limited supervision
  • Strong interpersonal skills
  • Collaborative and team oriented personality

Compensation: Commensurate with credentialing and experience

Job Type: Full-time

Pay: $36.00 - $38.00 per hour

Expected hours: 40 per week
