The Licensed Practical Nurse (LPN) position is responsible for the provision of quality nursing / health services necessary to maintain the health and well-being of individuals served while ensuring an expedient response to identified health needs within community homes. This position provides nursing coverage and support, including every other weekend. This responsibility includes planning, organizing, delegating, and monitoring nursing-related activities within the scope of the position. The level of involvement will vary based on the needs of each individual served as well as staff capabilities. The job responsibilities also include protecting and affirming the rights of the individuals by creating a healthy environment responsive to their needs. The LPN must be able to effectively support the nursing component of community homes. This position must be able to independently make decisions that impact the health and well-being of each individual’s life while following established nursing policies and procedures. The LPN must be able to communicate efficiently between LPN’s, Doctors, Coordinators, Individuals, Upper Management, Support Staff, Public Agencies, and Families.
The Reach Corporation is an ICF/IID Nursing Facility that has 6 to 8 bed homes out in the community. We teach our clients everyday living skills.
Job Types: Full-time, Part-time
Pay: $24.00 - $27.00 per hour
