A local corporation, family-owned Ambassador Health operates skilled nursing facilities in Lincoln, Nebraska City, Omaha and Sidney, Iowa, and a full-service home health agency that provides care to residents of southeast Nebraska and southwest Iowa.

Ambassador Health of Lincoln is centrally located close to hospitals and medical offices. The newly remodeled building offers spacious, private rooms in a safe and comfortable environment. The skilled nursing and therapy staff, focus on transitioning patients safely home from the hospital. Whether it’s a planned surgery, an unexpected illness or a traumatic injury, Ambassador Health provides in-patient physical, occupational, speech and respiratory therapy using a team-based approach focused on total-body wellness.

MDS Coordinator

Completes Minimum Data Set (MDS) on (1) new admissions, (2) assigned residents, and (3) any permanent significant and change in condition of resident

Notifies interdisciplinary team of need for completion of MDS upon a permanent significant change in resident’s condition

Visits and observes residents when completing MDS. Coordinates with staff and other resources (family members, physician, etc.) when providing care to the resident

Participates in morning report to identify new admissions, changes in resident condition, and any other resident concern.

Uses data contained in resident medical record when complete MDS focusing on information covering 90 days from previous reviewed MDS to include medication and treatment sheets, Activities of Daily Living, laboratory reports, and psychotropic medications.

Completes resident assessment protocol summary detailing concerns regarding resident, interventions attempted, success of interventions, and identifying areas in resident medical record where additional information may be found

Conducts review of restorative services provided to residents to determine need for particular skilled service; i.e., speech, physical, or occupational therapy.

Coordinates the preparation of the overall plan of care on assigned residents

Qualifications & Skills of our MDS Coordinator (RN or LPN):

  • Valid nursing license
  • AANAC RAC-T Certification for MDS
  • Advanced knowledge of EMR systems
  • Excellent communication skills
  • Working knowledge of health care laws and regulations
