Main Street Longevity, powered by 1st Responder Healthcare, is a modern, private-pay clinic focused on hormone optimization, metabolic health, peptide therapy, and performance-based medicine.
We serve both the general public and first responders, providing advanced wellness services, NFPA/WFI-aligned medical evaluations, cardiac risk screening, and performance-focused care.
Our model prioritizes lower patient volume, high-quality care, and long-term patient relationships.

The Role
This is an in-clinic provider role focused on delivering high-quality, relationship-based care.
Responsibilities include:
- Patient consultations and follow-ups
- Hormone optimization (TRT, estrogen, progesterone)
- Lab review and individualized treatment planning
- Peptide therapy, IV therapy oversight, and wellness services
- Participation in first responder screenings, occupational health evaluations, and performance-focused care
You will be the primary provider in clinic during scheduled shifts.
You will see up to 6–10 patients per day (typically fewer) with adequate time for charting and thoughtful care.
This is a quality-first environment—patients are not rushed through visits.
Ideal Candidate
We are looking for a provider who is:
- Confident and comfortable managing patients independently
- Able to build strong rapport and trust with patients
- Outgoing and personable in a private-pay setting
- Self-motivated and reliable
- Comfortable working in a small, growing clinic environment
- Interested in longevity, hormone optimization, and performance medicine
- Takes ownership of patient care and clinic responsibilities
- Professional and accountable in both care and documentation
Requirements
- Active Colorado NP license (please no PA applicants, must be able to work independently)
- Active Prescriptive Authority
- DEA license (or ability to obtain and maintain iby start date)
- Must maintain individual professional liability insurance
Preferred experience includes:
- Hormone optimization and longevity medicine
- Cellular medicine or peptide therapy
- Working with athletes and/or first responder populations

- $60–$75/hr (based on experience and fit)
