  • Educate patients on proper exercise techniques and self-care strategies.
  • Collaborate effectively with physicians, nurses, caregivers, and family members to ensure coordinated care.
  • Maintain accurate and timely documentation (no OASIS charting required for this role).
  • Set clear, achievable goals and track patient progress towards improved function and quality of life.

Qualifications:

  • Current Texas Physical Therapist (PT) license.
  • Doctor of Physical Therapy (DPT) degree preferred.
  • Minimum one year of experience in geriatric physical therapy (home health, ALF, ILF, or skilled nursing) highly preferred.
  • Valid Texas driver's license and CPR/BLS certification required.
  • Strong clinical reasoning and problem-solving skills.
  • Excellent communication and interpersonal skills to build rapport with patients and families.
  • Ability to work independently, manage time effectively, and meet documentation deadlines.
  • Compassionate and patient-centered approach to care.
  • Board certification in Geriatric Clinical Specialist (GCS) or Certified Exercise Expert for Aging Adults a plus.

Benefits:

  • Competitive hourly rate from $60/visit
  • Flexible scheduling options (PRN or part-time).
  • Positive and supportive work environment.
