Our integrated ONEcare Networks bring behavioral health, primary care, hospitals, social care providers, and payors together to create Uncommonly Good Collaborations. Our population health management services and technology improve outcomes for everyone by identifying disease early, prioritizing care, and building trust.
Unlike most conventional Clinically Integrated Networks and Accountable Care Organizations, all parties in the ONEcare Networks we create are incentivized and aligned to incentivize patient engagement and improve outcomes. ONEcare participants embrace hi-fidelity care models, close care gaps, anticipate crisis events, and provide transitional care support to mitigate clinical and social obstacles that inhibit treatment engagement.
