Job Summary
We are seeking a dedicated and experienced Case Manager to provide Supportive Services to residents of the Aid For Friends Emergency Shelter. The ideal candidate will possess a strong background in case management, supportive services, and grant compliance. This role requires effective communication skills and conflict management abilities. The Case Manager will also be occasionally responsible for overseeing case management for formerly homeless tenants in other Aid For Friends programs.

Case Management Duties

  • Oversee all supportive services and programs available for clients
  • Create daily shelter status reports and email them to the appropriate staff
  • Document all time spent with shelter clients, PSH tenants and community referrals
  • Conduct scheduled case management meetings with shelter clients, to include: assistance with Health & Welfare applications, applying for Social Security benefits, mental health, counseling, and setting up or finding transportation, distributing clothing vouchers, bus tickets and hygiene items as needed, contacting referral agencies on a regular basis to update services, encouraging tenants to attend available budgeting and life skills classes
  • Develop individualized, long-term housing goals with clients and monitor progress
  • Attend public and community outreach events and shelter presentations
  • Keep client files in compliance with IHFA and HUD requirements
  • Work closely with Shelter Manager regarding client written warnings and potential terminations
  • Work closely with Access Point to assist clients with housing eligibility documentation and placement preparation
  • Participate in ongoing training for supportive housing managers
  • Provide occasional administrative support as necessary, including phone etiquette, filing, and general office duties
  • When necessary, provide supportive services to permanent supportive housing tenants, including participating in scheduled home visits with PSH property manager.

HMIS Data Input

  • Enter client's information into HMIS and document services
  • Generate monthly data quality reports for shelter clients
  • Compile statistical information for grant applications and annual progress reports
  • Ensure that intake forms are current and updated

PERFORM OTHER DUTIES AS ASSIGNED BY THE CASE MANAGER SUPERVISOR AND/OR EXECUTIVE DIRECTOR

Qualifications

  • Licensed Social Worker or equivalent qualification is required. Equivalent qualifications include a Bachelor's in the area of human services with a minimum of 3 years experience in the field, a Bachelor's in any other area with a minimum of 3 years experience working in the field under the supervision of an LSW or equivalent, or Master's in human services with a minimum of 1 year experience in the field.
  • Familiarity with Microsoft Office (Word and Excel) and data entry into information management systems is preferred.
  • Strong knowledge of and experience working with local housing and mainstream resources is a plus.
  • Excellent motivational interviewing and problem solving skills with the ability to resolve disputes effectively.
  • Ability to build and strengthen professional relationships with landlords and partner agencies using professional communication and mediation.
  • Strong organizational skills with attention to detail for data entry and record keeping.
  • Ability to work independently as well as part of a team.
  • Must be able to climb a flight of stairs and to lift and carry 50 lbs
