Overview
Sound Decisions LLC is seeking a dedicated and compassionate GRO (General Residential Operation) Case Manager to join our team. The Case Manager will be responsible for coordinating services, supporting youth in care, maintaining documentation, and ensuring that each child receives the resources and support needed to achieve positive outcomes. The ideal candidate is organized, trauma-informed, and passionate about working with children and adolescents in a residential setting.
Responsibilities
- Conduct assessments and develop individualized service plans for youth.
- Coordinate and monitor services to ensure youth needs are being met.
- Maintain accurate and timely case documentation in accordance with state regulations and agency policies.
- Collaborate with families, caregivers, schools, therapists, probation officers, CPS caseworkers, and community providers.
- Participate in treatment team meetings and staffing meetings.
- Assist youth with educational, vocational, medical, and mental health service coordination.
- Advocate for youth and ensure their rights, safety, and well-being are protected.
- Monitor progress toward goals and update service plans as needed.
- Assist with discharge planning and transition services.
- Maintain compliance with Texas Health and Human Services Commission (HHSC) standards and licensing requirements.
- Participate in on-call duties and crisis intervention when necessary.
- Conduct regular assessments of resident well-being and adjust care strategies accordingly.
Skills
- Strong knowledge of care plans and the ability to develop tailored strategies for individual residents.
- Experience in social work or related fields is preferred.
- Proficiency in medication administration protocols within a residential setting.
- Familiarity with group home dynamics and best practices in dementia care.
- Financial acumen to manage budgets effectively while ensuring quality care delivery.
- Excellent management skills to lead a team of caregivers effectively.
- Nursing experience is beneficial, particularly in nursing home environments.
- A commitment to upholding resident rights and promoting dignity in care.
