Will be responsible for oversight of therapy performed in accordance with the interdisciplinary type. Our therapists are required to create a plan of care and evaluate, establish, and implement individual programs for our patients to increase functional ability and reestablish independence at home. Therapists have the option of utilizing assistants or continuing treatments.

This is an in-home/ field position (not telehealth) that has the ability to grow and be molded into your position of choice.

Duties & Responsibilities:

Performing home visits to evaluate patient's level of function

Per Medicare guidelines, developing an individual plan of care based on the client's personal goals

Implementing the treatment program through direct treatment of the patient - may use assistants or choose to perform treatments.

Reassessing clinical signs and symptoms to determine effectiveness of program

Instructing patient, family, and staff regarding participation in the treatment program

When necessary, collaborating with COTA and additional interdisciplinary staff member to improve treatment program

Valid driver's license and availability of a car
